We are thrilled to announce the addition of a second Florida Assertive Community Treatment Team (FACT) to our continuum of services. The FACT 2 Team has welcomed Mr. Nacin Fernandez, LCSW as its new Team Leader.
FACT is a multidisciplinary, mobile group of treatment providers consisting of a psychiatrist, psychiatric nurses, counselors, case managers, a vocational specialist, a substance abuse specialist, and a peer counselor, who offer 24-hour care to individuals with severe and persistent mental illnesses residing in Miami-Dade County. The team’s mission is to provide evidenced based, individualized services that help people meet their goals and stay in the community to avoid requiring more intensive services.
The new program manager, Mr. Nacin Fernandez, is a licensed clinical social worker with over six years of experience working with pediatric, adolescent, and adult patient populations. He is experienced in Cognitive Behavioral Therapy, Solution Focused Brief Therapy, Motivational Interviewing, and Grief Counseling Techniques. He is also fluent in English and Spanish and has been licensed as a clinical social worker in Florida since 2022. Mr. Fernandez’s impressive accolades include: Outstanding Leadership in SSWA and Phi Alpha Honor Society @ FIU, Up & Coming Award and the 2017 NASW Student Social Worker of the Year Miami Dade Unit. Mr. Fernandez’s experience and passion make him a great addition to the Fellowship House FACT 2 Team!
